ADULT DAY CARE: Centers are available for persons needing minimal assistance and social interaction with others. The current centers we have are listed in Orange County as Gentle Care. The average frequency for this program is four hours per day for five days per week. It is provided under Title III, Social Services Block Grant (SSBG), Choice, and Medicaid Waiver programs.
ATTENDANT CARE: A trained attendant comes to the home to assist the client with personal care and household chores. It is provided under Choice and Medicaid Waiver programs. Another option is Consumer Directed Attendant Care Program. This program offers the consumer, using CHOICE, as the payor for services, the option of using a provider of services willing to hire a caregiver of the consumer's choice. The amount of hours will vary depending upon the need. Tasks include household, minimal personal care, errands, and shopping.
HOME DELIVERED MEALS: Nutritious meals are brought to the home on a regular basis for those who are homebound. The average frequency is Monday-Thursday and they are delivered around lunchtime. The meals are delivered within a five mile radius from the Senior Center. There is an option of frozen meals if they are out of range. Diabetic meals are avaliable with a doctor's order. There is a suggested donation of $1.40 but is not required. Meals can be provided under CHOICE, Title III, and Medicaid Waiver programs.
HOME HEALTH CARE: Nurses and other home health care personnel visit the home to monitor unstable medical conditions and assist with personal care. Services which can be provided under CHOICE: Skilled Nurse, Licensed Practical Nurse, Home Health Aide, Physical Therapy, occupational Therapy, and Speech Therapy. Social Services Block Grant (SSBG) can provide weekly Home Health Aide visits.
HOMEMAKER SERVICES: A homemaker makes regular visits to clean the home. Hours may vary depending on need. No able-bodied persons should be living in the home. Service can be provided under Social Service Block Grant (SSBG), CHOICE, and Medicaid Waiver programs.
LIFELINE: An emergency response system worn by the client whereby he or she can notify medical personnel in the event of an accident or emergency in the home. Can be provided under CHOICE and Medicaid Waiver programs. They have the option of private paying for this service.
RESPITE SERVICES: This service gives individuals or families relief from their caregiving duties. Hours will vary upon need. This service can be provided under Social Services Block Grant (SSBG), CHOICE, and waiver programs.
TRANSPORTATION: This service assists clients in meeting their necessary transportation needs. Transportation is provided for visits to the doctor, grocery store, bank, and pharmacy. this can be provided under Social Services Block Grant (SSBG), Title III, and CHOICE.
OTHER SERVICES: These may include medical supplies and assistive devices such as wheelchairs, walkers, etc. CHOICE and waiver programs help to meet these needs.
