        Geriatric Care

    

     The Home Health/Hospice nurses understand the physical, emotional,

       and social changes that occur with aging.  They help the elderly plan

       care that promotes independence and quality of life in the home setting.

 

      Diabetic Management

 

       Nurses provide current information about diabetes care and assist the

       physician in implementing the most up-to-date interventions used in

       combating diabetic complications.

 

      Cancer Care

 

       Certified Hospice & Palliative Care Nurses answer cancer clients' specific

       questions about the the disease process, treatment, nutrition, and pain

       control while providing experienced care and support.

 

      Post-Hospital Recovery

 

     Hospital stays are shortened when home health nurses perform dressing

        changes, remove stitches or staples, instruct on post-operative self-care,

        or do other treatment and therapies in the home.

 

      Wound Care

 

      Our wound care program offers current wound care technology including

         photographic documentation, evaluation of the wound's underlying

         causes, response to treatment and clinical pathways that involve and

         instruct clients in self-care techniques.  Our nurses have also received

         training on the V.A.C., a state of the art wound care procedure.

 

      Enteral/Parental Therapy

 

      Nutrition, fluid replacement, antibiotic therapy, and pain management

         administered at home.

 

      Client Evaluation and Management

 

         Clients whose health and safety are in jeopardy due to multiple medical

         or complex care needs utilize skilled nursing services to evaluate and

         manage their care to promote maximum recovery and safety.

 

    Palliative Care

 

        This home health program emphasizes pain and symptom control for

         those facing life-limiting illnesses.  Clients may be pursuing curative

         treatment, have an extended prognosis, or otherwise be inappropriate

         for Hospice services.  This program is oversaw by the Hospice IDT and

         Certified Hospice & Palliative Care Nurses are available for consult.
