Quality Enhancement

Quality Enhancement, (QE), at Hoosier Uplands Home Health Care & Hospice, is an active, ongoing, interdisciplinary program encompassing all aspects of the agency.  This includes client care, clerical support, billing, training, and administration.

Hoosier Uplands Home Health Care Receives Quality Improvement Award

Hoosier Uplands Home Health and Hospice recently received a Medicare Home Health Quality Improvement Excellence Award, from Mitzi Daffron, RN, MSM, CPHQ, Executive Director, Medicare Quality Improvement Organization Health Excel, Incorporated.  "We would like to congratulate Hoosier Uplands Home Health Care for demonstrating commitment to providing quality health care to Medicare beneficiaries," she stated.

Hoosier Uplands Home Health Care also displayed perseverance and determination when striving for a common goal that will benefit the aging population of the community.

2005 IAHHC Spring Conference

 

At the IAHHC Spring Conference, Melissa Jeremiah, RN, Director of Operations for Hoosier Uplands Home Health Care, was presented with two awards from Sheila Cheek, RN, with Health Care Excel.  These awards were a Sterling Award for Improvement in Number of Surgical Wounds and an Excellence Award for Emergency Care Provided.  There were a total of 16 agencies that received a Sterling Award, eight agencies that received an Excellence Award, and three agencies to receive both a Sterling Award and an Excellence Award.  There are currently 202 Medicare certified home health agencies in the state of Indiana.  Melissa stated, "these awards would not have been earned without the planning of the OBQI Committee, which consists of Jody Brewer, RN, Quality Enhancement Director, Karen Galloway, RN, Assistant Director, Karen Cooper, HHA, and the follow-through of the field staff at Hoosier Uplands Home Health Care."

Health Care Excel Recognition

 

Health Care Excel (HCE), the Indiana Medicare Quality Improvement Organization, recently recognized Hoosier Uplands Home Health Care for our success in decreasing our acute care hospitalization outcome rates from a baseline rate of 28% or under and sustaining improvement based on review of the July 2006 OBQI data.  Hoosier Uplands Home Health Care was 1 of 22 such participating agencies, who met this goal, out of 205 medicare home health agencies in the state.

 

Patient/Family Satisfaction Survey

 

A Patient/Family Satisfaction Survey is sent to all patients upon discharge.  The family members or patient have the opportunity to evaluate their level of satisfaction with services.  Responses are reviewed at the QE and PAC meetings.

 

One of the goals this year was to maintain a client satisfaction level of above 95%.  This was met by maintaining a satisfaction level for 2007 of 98%.

 

Clinical Pathways

 

Clinical Pathways are standardized guidelines for delivery of care, based on diagnosis.  Hoosier Uplands personalizes care for each individual patient, and the "pathway" provides a reliable baseline for efficient quality care.

 

Ethics Committee

 

Hoosier Uplands Ethics Committee meets semi-annually, and as needed, to discuss ethical issues relating to the patient.

 

Corporate Compliance Committee

 

The Corporate Compliance Committee meets quarterly to ensure Hoosier Uplands Home Health Care & Hospice follow all Federal and State regulations and CHAP guidelines for service.
