- Daviess County Family Health Clinic
The Daviess County Family Health Clinic opened in January 2005 under Nurse Managed Clinic Funding from the Indiana State Department of Health. Funding for operations of this clinic are subcontracted to Daviess Community Hospital. The clinic also serves as a rural health clinic and health care professionals rural training site and is located in Washington, IN. Hoosier Uplands manages the current funding for this clinic which is Community Health Center funding of $150,000. The contract period for this funding is July 1, 2008 through June 30, 2009.
